-- Body Sculpting by Tumescent Liposuction Questions --
   
Q: What is liposuction and body sculpting?
A: Liposuction is a surgical technique that improves the body’s contour by removing excess fat from fatty deposits located between the skin and muscle. Body sculpturing by liposuction is literally a dream come true. Localized accumulations of fat, which are often hereditary and frequently impossible to eliminate by exercise or dieting, can now be removed permanently by liposuction surgery, never to plague you again.

Liposuction involves the use of a small stainless steel tube called a cannula. Connected at one end to a specialized suction pump, the cannula is inserted through tiny skin incisions. The removal of fat is accomplished as the suction cannula creates tiny tunnels through the fatty layers. After surgery, these tiny tunnels collapse, resulting in an improved body contour.

A: Tumescent liposuction using local anesthesia offers the benefits of fewer skin irregularities, less bleeding, reduced bruising, and a faster recovery.

Tumescent liposuction is performed in an office so the expense of hospitalization and the risks of general anesthesia are avoided. The procedure is much safer when only local anesthesia is employed. Because the patient is awake, he or she is able to sit or stand during the conclusion of the procedure, allowing the surgeon to check the status of the patient and the progress in achieving the desired contouring.

The surgeon may complete the procedure with the patient standing up, or remark the patient for additional suctioning on the procedure table.
A: Fat cells that are removed by liposuction do not grow back. As long as the patient does not gain an excessive amount of weight, the new, more pleasing silhouette is permanent.

A woman who always tends to put weight on her thighs and knees, will find that after liposuction, these areas are less likely than other areas to accumulate fat when she gains weight. In fact, a number of patients have had liposuction and subsequently gained weight. In these patients, the areas originally treated by liposuction generally maintain their new desirable silhouette, whereas areas not treated by liposuction are the sites of new deposits of fat. 
A: The Tumescent Technique of liposuction was developed in 1985. The Tumescent technique involves the use of large volumes of a dilute solution of lidocaine (a local anesthetic) in combination with the vasoconstrictive drug, epinephrine, which shrinks capillaries.

Local anesthesia used in the Tumescent technique for liposuction is so effective that patients no longer need intravenous sedatives, narcotic analgesics, nor general anesthesia. This technique is now considered the safest form of liposuction for removing very large amounts of fat because there is virtually no blood loss. Not only has the Tumescent technique proven to be safer than the standard technique using general anesthesia, but it also has proven to be less painful, has minimal post operative recovery time, and produces optimal cosmetic results.

With the Tumescent technique, infiltrating the local anesthesia is associated with minimal discomfort. Once the area has been completely numbed, surgery in that area is essentially painless. In addition, because the numbness remains in the treated area for more than 12 hours, there is no pain immediately after surgery.
A: Infections are extremely rare, regardless of whether or not the anesthetic technique is that of general anesthesia or local anesthesia. Although there have been reports in the medical literature of patients who have had serious infections following liposuction surgery, to the best of our knowledge, there have never been any patients who have had an infection following Tumescent liposuction performed using only local anesthesia.

Lidocaine, the local anesthetic that is used for the tumescent technique, kills bacteria. With the Tumescent technique the areas that are being treated surgically are those same areas that will receive the local anesthetic that kills bacteria. 
A: Liposuction



Basically we treat all patients with a laser to soften the fat preoperatively (which is used after the anesthesia so it is painless) and a special power vibrating cannula is used to facilitate the fat removal. Post operatively, vibro roller massages are offered along with radiofrequency body sculpting if needed.

A: BOTOX® Cosmetic is a miracle drug; a few tiny injections and years of age and wrinkles are removed in a few days. Specifically approved by the FDA for the treatment of frown lines between the eyebrows, standard treatments may include removal of forehead lines and treatment of Crow’s feet lines around the eyes. At Rockefeller Plaza Cosmetic MDs, we also provide innovative treatments with Botox including eyebrow lifts, lip and chin wrinkle removal, and neck band treatment.
A: The wrinkles in the facial area make us look old and tired. When injected beneath the skin, BOTOX® Cosmetic causes the muscles to relax, so that the skin is no longer puckered.

The wrinkles begin to disappear within a few days after treatment leaving the skin with a smooth appearance. The effects of BOTOX® usually wear off in 4-6 months.
A: RESTYLANE | SCULPTRA | RADIESSE | ARTEFILL | JUVEDERM
A: Restylane®is a safe and natural cosmetic dermal filler that restores volume and fullness to the skin to correct facial wrinkles and folds, such as nasolabial folds.

Restylane® is a cosmetic dermal filler made of hyaluronic acid, a natural substance that already exists in the human body. The hyaluronic acid in Restylane® is a crystal-clear gel called NASHA™ or Non-Animal Stabilized Hyaluronic Acid. NASHA is completely biocompatible with human hyaluronic acid.

Unlike rooster-derived hyaluronic acids and bovine collagen products, Restylane® is free from animal proteins. This limits any risk of animal-based disease transmissions or development of allergic reactions to animal proteins.*
A: Sculptra™ is a new type of cosmetic filler. It is a polymer of the naturally occurring substance lactic acid. It is approved by the U.S. FDA for replacement of fat loss in Aids patients, but cosmetic physicians may use it for replacement of volume in the fatty layer of healthy individuals.

When Sculptra™ is injected into the fatty layer, the particles attract the white bloods of the host. Collagen, elastin, and other structural substances are made which thicken the fatty layer.

How long do the results of Sculptra™ last?

Once the effect of Sculptra™ is achieved, it will last up to two years.
A: Radiesse® is an FDA approved cosmetic filler made of calcium based microspheres suspended in a natural gel, which is injected through the skin to correct smile lines, nasolabial folds and wrinkles around the mouth and nose. It is also used to provide volume in the cheeks and other areas which suffer from loss of volume due to natural aging.

The calcium base microspheres work by providing volume correction to eliminate defects and wrinkles upon injection. Over time, the reaction to the microspheres causes the body to synthesize additional collagen which gives further correction to the original problem.

How long does it last?

A: Mesotherapy is a painless technique of injecting a small amount of a customized mixture of conventional pharmaceutical or homeopathic medications, vitamins, minerals, amino acids and enzymes into the mesoderm - or middle layer of skin.

These very small doses of the substances are administered using very tiny needles in specific areas depending on the condition being treated.

Mesotherapy is used to treat a broad range of illnesses and pain conditions. This minimally invasive approach has shown successful results in many areas: sports injuries, hair regrowth, alopecia, and cosmetic treatments.
A: Mesotherapy may be used for cellulite, body fat reduction, body sculpturing and weight loss, facial rejuvenation (mesolift), and eye fat pads. All of these uses have shown good results, but only after a number of treatments have been performed
A: The office procedure takes less than 30 minutes per session and requires little preparation or recovery time.

No heavy bandage or compression girdles are required. The majority of patients experience minimal discomfort with mesotherapy. Some patients may develop minimal local pain, a minor burning sensation for a few hours, and temporary bruising. Itching and soreness may be experienced for a few days.

Like any other injectable product, an allergic reaction may occur in some people. The procedure is performed in the office and takes minutes to complete.
A: Yes. Mesotherapy is a non-surgical medical treatment. Mesotherapy removes the fat from the fat cell, where liposuction removes the entire fat cell. Liposuction is a surgical procedure that requires anesthesia (local, regional, or general). Mesotherapy does not require anesthesia nor recovery time. However, mesotherapy is not a replacement for liposuction.

Mesotherapy is applicable for the reduction of small areas of unwanted fat spots, cellulite and skin rejuvenation. Mesotherapy is best suited for men and women who are not excessively overweight but, instead, have certain problem areas with stubborn fat that can no longer be reduced by diet and exercise. These patients want liposuction but are hesitant to undergo a surgical procedure. The downside with mesotherapy is mainly that the results are gradual when compared with the immediate results that liposuction offers.

Mesotherapy has shown excellent results to smoothen and reduce uneven fat spots that may persist after liposuction, thereby avoiding the need for additional touch-up liposuction procedures. Mesotherapy and liposuction are not competitive procedures but, rather, they complement each other

A: Photorejuvenation is a procedure intended to rejuvenate the skin on the face, arms, legs, neck, and chest. It is performed in a series of treatments of intense pulsed light, or IPL.

It is the process of using light on the skin’s surface to effect changes beneath the surface of the skin. Blood vessels, brown pigment, hair follicles may be removed and wrinkles may be reduced with this treatment.
A: Photorejuvenation is performed with a series on Intense Pulsed light treatments. The light penetrates the top layer of skin and acts upon the underlying layer of the dermis.

The light energy seals small blood vessels, destroys melanin pigment, destroys hair follicles, and stimulates cells to produce new collagen which reduces wrinkles.
A: Results should become especially noticeable after several treatments, as wrinkles, redness, and age spots fade away.

Patients also enjoy a reduction in pore size, and an overall better skin tone and texture. There is virtually no downtime associated with photorejuvenation.

Some patients experience slight redness after their IPL treatment, always lasting less than 24 hours.
A: The same side effects as laser | IPL hair removal or vein treatment, (i.e. possible slight burning, blistering, and extra pigmentation on the skin).
A: After a series of 3-4 treatments, the improved appearance should last for 1-2 years.
 
 
Fractional Technologies
 
A: This is a new technology which uses deeply penetrating laser beams interspersed with areas of no treatment, in a grid type pattern. The result is well controlled deep dermal tissue destruction which the body can heal, without significant down time
A: This technology allows for the treatment of acne scarring, deeply seated pigment (melasma), surgical scars, and fine and moderate sized wrinkles around the eyes, mouth, and other areas.
A: Usually 2-6 treatments.
A: No, most patients do not require anesthesia and complain of only minimal sticking pains during the treatment.
A: No. There is a little swelling and redness for a few hours up to one or two days, however, there is no limitation on resuming any activities.
 
 
Infrared Tissue Tightening
 
A: Tissue Tightening is a new procedure involving the use of infrared light to deeply heat the tissues in the dermis and subcutaneous space. The result is a tightening of the skin which continues to improve over the next 6 months
A: Light is produced in the infrared range of wavelengths designed to heat the non-pigmented deeper tissue of the skin. Collagen fibrils are denatured and shortened causing a contraction of the dermis over time

Because the tightening effect is spread out over the whole area there is no distortion of the tissue as is sometimes seen with surgical tightening procedures
A: The treatment takes 30-60 minutes depending upon the body site which is treated. It is essentially painless, although some patients report a warming sensation. There is no down time and patients can resume full activities immediately.
A: Infrared light can treat lax or sagging skin. It is commonly used in the face and neck areas but can be used anywhere on the body, including the belly, arms and legs.

It has been called a "facelift without surgery".
A: No procedure should be viewed as completely permanent. Skin tightening obtained from the tightening procedure should last for years.

You may require up to 3 treatments approximately 1 month apart in order to see the expected results.

One way to reduce the effects of aging is to avoid exposure to UV light (or sun).

This is a great procedure for anyone who is not ready or willing to have a surgical facelift. The results are natural with overall tightening which does not distort facial features.

A: Bipolar radiofrequency gives superficial heating to the dermis which causes collagen remodeling leading to a reduction in wrinkles and tightening of the skin.

Unipolar radiofrequency provides deep heating down to the fatty layer leading to body contouring and reshaping.
